The Editor,
Our article on off-pump coronary artery bypass grafting (CABG) and Dengue fever [1] highlighted the possibility of dengue hemorrhagic fever to be kept in mind while dealing with the patients of postoperative thrombocytopenia. The reference to the same by Wiwanitkit is welcome and highlights the rare causes of thrombocytopenia. [2] Chikungunya fever is common in southern India and dengue hemorrhagic fever is more common in northern India. As the patient in the case report was operated in a hospital in north India, going by the medical practice of finding the common medical causes first then the rarer ones, dengue serology was sent which was positive. Secondly the incubation period of dengue hemorrhagic fever is 4-7 days. [3] At our institution patients are admitted for elective CABG one day prior to the surgery and this patient was asymptomatic at that time. This patient developed symptoms of dengue hemorrhagic fever on third postoperative day and therefore it does not appear to be a nosocomial infection. It goes without saying that our institution has infection control and mosquito control at par with international standards and thus the possibility of mosquito bite in our hospital is very low.
