The Editor,

We appreciate the interest of the authors about our article titled, "Acute hemodynamic effects of inhaled nitroglycerine, intravenous nitroglycerin, and their combination with intravenous dobutamine in patients with secondary pulmonary hypertension." [1]

The study was indeed designed to be conducted in patients undergoing elective mitral or double valve surgery associated with pulmonary arterial hypertension (PAH), defined as mean pulmonary artery pressures (mPAP) > 25 mmHg. Patients with mPAP< 25 mmHg in the period after termination of the cardiopulmonary bypass were to be excluded from participating in the study. From the footnote of table 2, [1] showing the hemodynamic parameters, it is clear that the values of mPAP are given in median (range) pattern and not in mean ± SD pattern. That is why in some groups the median values are less than 25 mmHgThus, it appears that the author's assumptions were incorrect.

We agree with the author's concerns about inhaled nitroglycerine being more beneficial than intravenous nitroglycerine in patients with right ventricular dysfunction secondary to PAH. This aspect of the study needs further investigation.
