The Editor,
I read the recent report on post-operative management of endarterectomy with great interest. [1] Iyengar et al, concluded that "Pulmonary endarterectomy is an effective and potentially curative surgical treatment for patients with severe chronic thromboembolic pulmonary hypertension." [1] Of interest, this alternative approach for the cases unfit for pulmonary thrombectomy might be a good one. However, there are also several factors determining the outcome of chronic thromboembolic pulmonary hypertension, including splenectomy, permanent central intravenous lines and certain inflammatory disorders. [2] .In addition, in the report by Iyengar et al, there are some points of concern. First, the nature of the retrospective study might pose some bias and the numbers of subjects might be a major limitation of the generalization of the results. Additionally, the novel pharmacotherapy that is recommended for chronic thromboembolic pulmonary hypertension, Bosentan, should also be mentioned. [3] The comparison between this medical approach and pulmonary endarterectomy can give useful clinical information.
